NHS FREEDOM OF INFORMATION PROJECT BOARD MODEL PUBLICATION SCHEME FOR COMMUNITY PHARMACISTS

 

Welcome to the Field Pharmacy Publication Scheme. This is the Publication Scheme for the registered pharmacists, Reg Kirby and others, who practice within Field Pharmacy at the following branch;

11 Old Road, East Peckham, Kent, TN12 5AS.

It is about the NHS services, which the Pharmacists provide, as required by the Freedom of Information Act 2000.

 

Introduction

 

This Publication Scheme is a complete guide to the information routinely made available to the public by our registered Pharmacists. It is a description of the information about our Pharmacists and Pharmacies, which we make publicly available. It will be reviewed at regular intervals and we will monitor its effectiveness.

 

How much does it cost?

 

The publications are all free unless otherwise indicated within each Class.

 

How is the information made available?

 

This document may be viewed on the Field Pharmacy website, www.fieldpharmacy.co.uk, or a hard copy may be ordered from the East Peckham branch.

 

Your rights to information

 

·        In addition to accessing the information identified in the Publication Scheme, you are entitled to request information about Field Pharmacy under the NHS Openness Code 1995.

·        The Freedom of Information Act 2000 recognises that members of the public have the right to know how public services are organised and run, how much they cost and how the decisions are made.

·        From January 1st 2005 it will oblige all Pharmacies to respond to requests about information that it holds about NHS Pharmaceutical Services, and is recorded in any format and it will create a right of access to that information. These rights are subject to some exemptions, which have to be taken into consideration before deciding what information it can release.

·         New environmental information regulations may be introduced as early as 2003. These will enable similar access to environmental information as under the Freedom of Information Act 2000.   

·        Under the Data Protection Act 1998, you are also entitled to access your patient medication records or any other personal information held about you. This can be done via a written request to Reg Kirby, Superintendent Pharmacist, through the East Peckham branch.

CLASSES OF INFORMATION

 

All information at Field Pharmacy is held, retained and destroyed in accordance with NHS guidelines. Our commitment to publish information excludes any information, which can be legitimately withheld under the exemptions set out in the NHS Openness Code or Freedom of Information Act 2000. Where individual Classes are subject to exemptions, the main reasons are e.g. the protection of commercial interests and personal information under the Data Protection Act 1998. This applies to all Classes within the Publication Scheme. The information on this Scheme is grouped into the following broad categories: 

 

1. Who we are

Class Description: Details of the practice, organisational structures, key personnel and how we fit into the NHS

 ·       Field Pharmacy, East Peckham branch, operates under the Maidstone Weald  Primary Care Trust.

·        The Primary Care Trusts have made arrangements with Field Pharmacy for the provision of pharmaceutical services under section 41 of the National Health Act 1977. The Pharmacy is contracted to provide a range of NHS services. (A full list of these will appear below in Class 2).

·        Local Pharmaceutical Services Scheme. Under the Health and Social Care Act which amends the NHS Act, arrangements have been made for the establishment of Local Pharmaceutical Services Schemes by a Primary Care Trust. First wave pilots have been announced but have not yet gone live. Regulations are in place. Field Pharmacy or the registered Pharmacists will be providing Local Pharmaceutical Services under a Local Pharmaceutical Services Scheme established by their Primary Care Trust.   

·        Field Pharmacy is the trading name of M.E.F. (Rx) Ltd.

·        The Field Pharmacy Superintendent Pharmacist is Reg Kirby.

·        Reg Kirby (Registration No 63782) is the manager of the East Peckham branch.

·        Field Pharmacy also employ locums.

Some information will be withheld, including personal, confidential information about individuals that is protected by the Data Protection Act.   

 

2. Our Services

Class Description: The range of services we provide under contract to the NHS

 

·        The NHS pharmaceutical services which Field Pharmacy are contracted to provide with the relevant Primary Care Trust to NHS patients will be outlined here such as:

-Dispensing of NHS prescriptions for drugs, medicines and Drug Tariff listed appliances

-Record keeping for dispensed medicines

-Extra NHS services e.g. provision of oxygen

-Repeat prescriptions

-Residential and nursing homes

-Safe disposal of unwanted medicines

-Repeat dispensing (as part of the repeat dispensing pathfinder scheme)

-Minor ailments schemes

-PGD’s

 

·        Listed below are the services that we may provide for which there is no NHS remuneration:

- Advice from the Pharmacist

- Prescription delivery service

- Confidential help and advice

- Emergency supplies

- Smoking cessation support

- Dispensing of private and veterinary prescriptions

- Provision of emergency hormonal contraception

 

·        Field Pharmacy Practice Leaflets, outlining the full range of services provided, may be obtained from our East Peckham branch.

·        Field Pharmacy’s inclusion on the List of the relevant Primary Care Trust for each branch, is subject to Terms of Service. The Terms of  Service for Chemists can be obtained directly from the Primary Care Trust.

·        Policy for data protection and patient confidentiality, with regard to computerised / handwritten patient records relating to prescribed medicines are undertaken in accordance with our Standard Operating Procedures.

3. Financial and funding information

Class Description: Funding details and charging policies

 

·        Information about charges for prescriptions, who is exempt and prepayment certificates can be obtained from any of our branches.

·        Prescription charges are paid directly to the NHS not to Field Pharmacy. Information on the average cost of a dispensed item and the dispensing fee that Field Pharmacy receives for each dispensed item are available from any of our branches.

·        There may be circumstances where material cannot be released because it is confidential or commercial information or the appropriate officer designated for these purposes under the Act has taken the view that it may be prejudicial to the conduct of the Pharmacy’s affairs.

 

4. Information about Policies, Prescribing and Prescriptions

Class description: Protocols in relation to prescribing protocols and standard operating procedures relating the dispensing of prescriptions, data protection

 

·        Field Pharmacy operate under agreed protocols and standard operating procedures relating to dispensing of prescriptions, copies of which are available from Reg Kirby via our East Peckham branch.

·        Policies in relation to the above

 

5. Regular publications and information for the public

Class description: Guidance and information leaflets

·        Field Pharmacy offer a range of free patient information leaflets. These will vary throughout the year as and when published by a variety of sources mostly from non-public funds. 

·        Information received from the Primary Care Trust that needs to be passed on to the patient

 

6. Complaints

Class description: Policies, procedures and contacts for complaints

 

·        The complaints policy about the NHS services provided by Field Pharmacy, are outlined in our standard operating procedures. The complaints policy is in line with the Pharmaceutical Services Regulations and the NHS complaints policy. If any problem cannot be resolved within the Pharmacy or if any member of the public does not want to raise it directly with the Pharmacy, he / she can contact the relevant Primary Care Trust. 

  

7. This Publication Scheme

Class Description: In this class we will publish any changes we make to this Publication Scheme, the criteria on which our information management policies are made and a referral point for all enquires regarding information management generally in the Dental Practice. We will also publish any proposed changes or additions to publications already available.

 

Cost of Information

 

All information is currently made available free of charge, however, we reserve the right to alter this policy at any time in the future.

 

Any charges made will be reviewed regularly.
